Dementia care model that reduces hospitalizations successfully translated into practice

INDIANAPOLIS -- An innovative model of dementia care that significantly reduces emergency department visits and hospitalizations, and encourages use of medications that are not harmful to older brains, has now been used to treat over 1,000 patients. The Aging Brain Care model was developed by researcher-clinicians from the Regenstrief Institute and the Indiana University School of Medicine.

Extending the definition of a patient to include family members who enable cognitively impaired individuals to live in the community, physicians, nurses, social workers and other staff members work closely with both the older adult and family caregivers -- in the exam room and in the home, as well as over the phone and via email -- to deliver care to improve both brain and physical health.

This month, the 1,000th patient was seen at the Wishard Healthy Aging Brain Center, the first facility to use the ABC model. The Healthy Aging Brain Center is both a research lab and a treatment facility focused on the mental status of elder adults. The center has seen reductions of 45 percent in hospital emergency department visits and 54 percent in hospitalization stays in patients compared to similar individuals not seen in the center.

"Patients treated utilizing the ABC model have fewer behavioral and psychological symptoms of dementia after one year than they had at the onset of treatment," said Regenstrief investigator Malaz Boustani, M.D., associate professor of medicine at the IU School of Medicine and the medical director of the Wishard center.

Patients receiving care in the Healthy Aging Brain Center are given an initial cognitive assessment including neuropsychological testing, an MRI, a medication review and a structured neurological and physical evaluation. The staff then helps both patient and caregivers develop a personal treatment plan that typically includes recognizing potentially harmful medications, prescribing new medications, initiating brain and physical exercise regimens, and working on reducing stress to improve daily life.

"Our research over the past decade has shown the importance of families and communities, in addition to medical care, in improving the quality of life for older adults with dementia," said Regenstrief investigator Christopher Callahan, M.D., Cornelius and Yvonne Pettinga Professor in Aging Research at the IU School of Medicine. "The HABC uses a team-based approach to help get everyone on the same page in meeting the goals of care for the patient and their family caregivers."

Dr. Callahan sees patients at the Healthy Aging Brain Center and is also founding director of the IU Center for Aging Research.

The Healthy Aging Brain Center receives referrals from across the country and has quickly become a nationally recognized leader in the care of older adults, according to Dr. Boustani, a geriatrician.

Earlier this month, the new Senior Health Implementation Center at the Regenstrief Institute announced that it will disseminate ABC tools, including a how-to manual for clinicians, a dementia symptoms monitor and a resource handbook for caregivers, as well as provide onsite training.

According to the Alzheimer's Association, an estimated 5.4 million Americans -- one in eight older Americans -- have Alzheimer's disease.

In an effort to improve their secondary, the Atlanta Falcons shipped merely a seventh-round pick to the Eagles for cornerback Asante Samuel.? So far, it looks like the Falcons pulled off the kind of steal for which the Eagles had become notorious.

According to Dave Campbell of the Associated Press, Samuel made a real impression in the Falcons? first full-squad practice of the 2012 offseason.

?Asante I thought did about the same thing he has done to me for four years,? quarterback Matt Ryan said. ?He made a good pick.? He?s going to make us better.? Specifically I think he?ll make myself and our wide receivers better going against him and Dunta [Robinson] and [Brent] Grimes every day as corners.? It?s going to be good work for us.?

Robinson thinks Samuel?s energy may have surprised some members of the offense.

?It?s a lot of fun,? Robinson said. ?It makes practice fun.? All the talk, all the yelling.? I can tell the offensive guys weren?t very used to it.? They didn?t know how to take it.? But it was a lot of fun.

Updated 7:45 ET: SEATTLE -- At least four people were killed and two others wounded in two shootings minutes apart Wednesday, police said -- bringing the number of homicides in Seattle this year to nearly as many as in all of 2011.

Police said the man suspected of the shootings -- at a north Seattle cafe and in a downtown parking lot -- has died of a self-inflicted gunshot.

The man, who fit the suspect's description, knelt down in the middle of the road and shot himself upon seeing a uniformed officer approach, Assistant Police Chief Jim Pugel said.

It was unclear throughout the day whether the two shootings were connected but Pugel confirmed that they were during a news conference on Wednesday afternoon. He said the similar caliber of the bullets have led police to that conclusion.

Police confirmed that two male victims were dead at the scene of a shooting at a north Seattle cafe at about 11 a.m. Two more men and a woman were taken to the hospital in critical condition. The woman died at the hospital, NBC station KING5-TV reported.

The shooting occurred at the Caf??Racer, a restaurant and music venue in a commercial district near the sprawling University of Washington campus. Roosevelt High School, four blocks from the incident, was on lock down as police looked for a suspect.

About a half hour later, some five miles to the south, a woman was shot several times during what police initially said was a possible carjacking at a parking lot next to Town Hall Seattle, a community cultural center in downtown Seattle.?Police found the vehicle, a black Mercedes SUV, and a handgun miles away in a West Seattle residential neighborhood.

Even before the shootings on Wednesday,?Seattle was dealing with an outbreak of violence that has stumped police officials, who have blamed it on gangs and weapons.

Two men were killed and three more people were seriously wounded in a shooting at a Seattle cafe by a gunman who fled the scene on foot. KING's Linda Brill reports.

At a City Council meeting on Tuesday, police put the number of homicides in the city at 15, compared to 21 homicides for all of 2011, according?to a report in The Seattle Times.?

Last Thursday, Justin Ferrari, a 43-year-old software engineer, was gunned down while running errands with his children in the Central Area. No arrests have been made in that investigation. And over the holiday weekend, four-drive by shootings, one that left a bystander wounded, were attributed to gang activity.

Police have not identified the suspects in either incident on Wednesday, though a picture of the suspected shooter at the cafe was released late in the day. ?He was last seen running away and remains at large.?The suspect is described by police as?a white male, about 30 years old, wearing a dark short cap and a light blue jacket with well-groomed dark hair and a neat full beard.

One law enforcement source told KING5 that the shooting could be an act of domestic violence.

Evan Hill, who lives above the building where the cafe shooting happened, said the cafe was an artists' collective and performance space.?

"It's the strangest place to think of a shooting," Hill, who heard four to five shots, told The Associated Press. He said he ran to his balcony and called 911, but didn't see a suspect.

At the Trading Musician store next door, store manager John Herman said they didn?t hear any sounds until police converged on the scene.

?Our first customer of the day had just come from Caf? Racer, and he thought he walked out right as the person walked in,? Herman told msnbc.com. ?He thought he saw the (shooter) but other witnesses say that he may have actually seen a victim.?

Trading Musician employees hang out at Caf? Racer, Herman said, and the store holds its holiday parties there. Herman said other Caf? Racer employees had arrived and were standing across the street. The owner had also arrived and had been escorted by police to the crime scene, he said.

The Emerald City, generally considered a safe place to live, had 19 murders already this year. Last year there were 20. NBC's Mike Taibbi reports.

?They seem very upset, but I don?t know if it?s about the fact that it happened, or because they lost somebody,? he said. He said the Trading Musician was effectively closed, but that employees hadn?t left.

?We?re just waiting to find out what has happened to our extended family,? Herman said.?

The suspect in the parking lot shooting is also on the loose and is considered armed and dangerous, according to police. He was described as a white male, about 35 years old, blonde haired with a crew cut.?

Live video from a KIRO television helicopter showed heavily armed officers following a police dog through a residential neighborhood.?

Ibrahim Frishak, who was power-washing a sidewalk across the street from where the carjacking occurred, said he heard gunshots and saw a car peeling away,?The Stranger, an alternative weekly newspaper, reported.?

"I just got back a week ago from Libya on vacation to visit relatives," Frishak told the paper. Everyone over there carries guns, he said, adding, "Now I think Libya is safer than Seattle."

At an afternoon news briefing, Seattle Mayor Mike McGinn said he told police that their "highest priority is to find the shooters and bring them to justice." He also asked political leaders to look into gun laws and the culture of violence to see what can be done to keep weapons out of the hand of offenders.

This is harder than it sounds with the economic interests of countries like China and India making things especially difficult with a memorable climax at the climate conference at Copenhagen in 2009. But sadly, Nasheed did not have that much time as he was ousted in a threatened coup in February of this year.

Most value propositions for new businesses come from some fundamental trend in the economy, in demographics, in technology or in society and culture.? These trends lead to changes within industries.

For example, the widespread use of the Internet forever changed industries such as music and newspapers.? Inflation in the economy both led to soaring healthcare costs and more recently begun to affect the food industry. ?Inflation has clearly shaken up both of these industries.

A fundamental role of being an entrepreneur is to find solutions for the problems and needs customers have that result from the change that follows disruptive trends like these.

Once the entrepreneur identifies the new business opportunity, the next step is to identify the specific offerings that provide enough value to customers to get their attention and motivate them to purchase from the new business.

It could be something about the product itself, such as its price and value, features, performance, durability or design. ?It could also be something about how the product or service is delivered to the customer, such as its method of delivery (in their homes, on-line, in an exciting new retail location, etc.) or the specific services offered to go along with the product.? Or, it might be something about the personnel of the company, including their expertise, responsiveness, or reliability.

It is almost always best to identify and focus on one or two things that will make your business stand out to customers.? Focus on the most important need or problem the customers are facing.? Offer that feature to the customers with excellence in mind, making sure all employees understand its importance.? And make that key feature the heart of all of your promotion and other communication with the customer.

The best way to develop the key features that are at the heart of your value proposition is to listen to your customers, as while you may think you know what the customer wants, most of the time you will not have it quite right.? You will probably have to adjust your product or service to fit with what the customer actually wants or needs.

You may have started with something that is too complex and confusing to the customer.? What you thought would be one of many features of your product may need to become the only thing you focus on.

Sometimes the entrepreneur starts too narrow, and what was thought of as the product or service is only one of its key features ? you may need to broaden what it is you offer.

Or you may discover that what you offer is a great value, but you have not been selling it to the right target market.

Once your customers affirm that you are offering the right value proposition, it needs to become the focus of everything you and your employees do every day.

Garlic constituent blocks biofilm formation, could benefit cystic fibrosis patients and others

ScienceDaily (May 27, 2012) ? E Pluribus Unum, the de facto motto of the United States, could just as well apply to biofilm-forming bacteria. Bacterial biofilms are far more resistant than individual bacteria to the armories of antibiotics we have devised to combat them. Now Tim Holm Jakobsen and Michael Givskov of the University of Copenhagen, and their many collaborators have pinpointed a constituent of garlic that attacks a key step in the development of biofilms, in an effort they hope may offer help in particular for patients with cystic fibrosis.

The research is published in the May 2012 issue of Antimicrobial Agents and Chemotherapy.

In earlier work, Givskov and his colleagues showed that "crude extracts of garlic inhibit the expression of a large number of genes that are controlled by bacterial quorum sensing [communication among bacterial cells involved in biofilm development], and that extracts promote a rapid clearing of pulmonary Pseudomonas aeruginosa infection in mice," he says. "These findings encouraged us to identify and assess the efficacy of the pure active compound."

That compound turned out to be ajoene, the major constituent of a multitude of sulfur-containing compounds produced when garlic is crushed, says Jakobsen. The team then showed in P. aeruginosa that ajoene inhibits expression of 11 genes that are controlled by quorum sensing. "These key genes are regarded as crucial for the ability of P. aeruginosa to cause disease," he says.

"We also found ajoene to reduce the production of rhamnolipid, a compound that shields the biofilm bacteria from the white blood cells that otherwise would destroy bacteria, and that by combining ajoene with the antibiotic tobramycin, it was possible to kill over 90 percent of bacteria living in a biofilm," says Jakobsen.

"Our study is part of a series of comprehensive investigations of natural compounds targeting bacterial quorum sensing systems, and it further strengthens previous proof of concept research we conducted on the potential of compounds which block communication among pathogen cells in contrast to simply killing bacteria, as conventional antibiotics do," says Jakobsen. Such alternative approaches "may postpone or minimize development of antibiotic resistance," he adds.

Jakobsen says the garlic project grew out of a major donation from the German Cystic Fibrosis Association. "In CF patients, P. aeruginosa infection leads to bronchieciasis, pulmonary fibrosis, respiratory failure, and death," he says. "Despite intensive antibiotic traatment, CF patients have a life expectancy of about 40 years, and the main cause of death in CF patients remains complications associated with [this infection]." Jakobsen's team and the German CF Association have patented the action of ajoene against biofilms, and are seeking a pharmacutical partner to develop antimicrobial drugs based on ajoene.

Jakobsen notes that garlic has been used medicinally "for thousands of years." Garlic not only has antibacterial properties; it has anti-viral, anti-fungal, and anti-protozoal properties as well, and it has beneficial effects on the cardiovascular and immune systems, as well, he says.

VATICAN CITY (AP) ? The Vatican has confirmed that the pope's butler was arrested in an embarrassing leaks scandal.

Spokesman the Rev. Federico Lombardi on Saturday said Paolo Gabriele was arrested in his home inside Vatican City with secret documents in his possession. Gabriele, a layman, was being held.

Vatican documents leaked to the press in recent months have pointed to power struggles and accusations of corruption touching senior Vatican cardinals. In one case, a cleric who complained was reportedly sent to Washington as papal envoy to get him out of the Vatican.

ZURICH (Reuters) - UBS AG (VTX:UBSN.VX - News) could see up to 10 percent of its European assets of 300 billion Swiss francs ($312.52 billion) moved out due to pressure to clamp down on untaxed accounts, the head of the wealth management business was quoted as saying on Saturday.

"We have been losing assets in Europe for many quarters, around 10 billion francs to date," Juerg Zeltner told the Finanz und Wirtschaft newspaper in an interview.

"And we expect further asset outflows going forward, in the region of 12 to 30 billion francs. That's a bitter pill to swallow. We are able offset this erosion thanks to strong growth in new markets."

Zeltner said UBS had assets under management of over 300 billion francs in Europe, over 170 billion francs in Asia, around 110 billion francs in emerging markets and 130 billion in Switzerland.

"Banks that are focused largely on the European cross-border business are having a much tougher time," he said.

After mounting criticism of its tradition of strict bank secrecy, Switzerland has agreed to do more to help foreign governments hunt tax evaders and has signed deals with Germany, Britain and Austria to levy punitive taxes on undeclared assets held in its banks and a withholding tax on future client income.

Zeltner said he did not expect a mass exodus of clients once the deals come into force, in theory in 2013, although the German deal still faces some opposition from the Social Democrats.

"The lion's share of the German money leaving the bank will go to the tax authorities. There is no evidence of clients taking their money out of the country as they did in the U.S. case," he said.

UBS paid a big fine and handed over details of more than 4,000 clients to U.S. authorities in 2009 to settle allegations it helped wealthy Americans dodge taxes.

"I see no indication that assets are being moved to Singapore or Hong Kong," Zeltner added.

UBS posted strong money flows into its flagship private bank in the first quarter, underscoring its appeal as a safe haven for the savings of the world's wealthy.

Switzerland's biggest bank, scaling back its risky investment bank which suffered huge losses in the credit crunch to focus on private banking, said the unit won a net 6.7 billion francs in fresh assets and hiked margins.

UBS is also trying to recover from a rogue trading scandal involving London-based Kweku Adoboli, accused of unauthorized deals that cost the bank $2.3 billion.

Gaming has been an on and off hot topic for researchers over the past few years for a handful of reasons. Do they help or hinder a developing mind? Can they truly push us into committing violent acts? The effects they have on the minds of the player has been debated constantly, too.

Surprising to some, studies are showing that gaming does actually have a number of benefits. Yes, there are obvious negatives that go along with it hand-in-hand, but it?s also awesome to see that gaming can be seen in a more positive light by proof of SCIENCE!

One of the most recent studies was conducted by MSU (Michigan State) professor Linda Jackson,?whose expertise lays in psychology. Her research included 491 12-year-olds and she studied the link between video games and their creativity. One of the most relied on tests, the ?Torrance Tests of Creative Thinking?, was used in an attempt to measure the children?s creativity. Basically, the tests show a child a basic drawing and it?s up to them to determine what it is that they see. A dot that has many lines stemming from it could be responded to with things such as: a squished spider, a star or a set of mini blinds caught in a tornado. From there, children are asked to write stories based on that said shape.

The results of the test were very clear; most of the children that played video games created ?interesting and exciting? work while the others did not. Surprisingly, the type of game played and whether the game was violent didn?t matter. No correlation was found in other technological factors like cell phone, Internet, and computer usage.

The MSU findings should motivate game designers to identify the aspects of video game activity that are responsible for the creative effects, Jackson believed.

?Once they do that, video games can be designed to optimize the development of creativity while retaining their entertainment values such that a new generation of video games will blur the distinction between education and entertainment,? Jackson continued.

Personally, I?m not shocked by these findings. Games open whole new worlds to us, full of all sorts of wonders. Inspiring art and music also helps. I believe having children exposed (within reason) on a regular basis would definitely help to get their creative bug going.

I?ve been gaming for as long as I can recall and I?ve always been considered a creative person. As twisted as it may be to say, Capcom?s Resident Evil 2 was my muse as a child. I loved to play make believe as if I were in the devastated Raccoon City, draw pictures from the play manual, and I wrote numerous short stories about the game.

As I?ve always said, most of the effects video games have can be helpful or hindering depending on the child?s environment. I believe games will always help grow a creative mind, but a child?s mind could also be easily swayed. For a child to have a healthy sense of make-believe and reality, it?s up to the parents to re-enforce what?s real and what is not, as well as?what?s acceptable and what is not.

Games have taken a lot of flack; they?ve been cast as the bad guys who warp impressionable minds. I think that?s ridiculous. Which games are they claiming have such a terrible sway on children? Mostly violent ones that shouldn?t even be in their hands, anyways. I can?t even begin to say how many parents came in and purchased games like Grand Theft Auto for their kids even after constant warnings from me of their rating and content. It?s the same parents who claim their child ?can handle the violence? or ?knows better than to mimic the game? who cry wolf as soon as trouble brews.

In my eyes, it?s more so poor parenting that can lead to unsavory results when children and gaming are in the mix, not the games themselves. I suppose it?s easy to forget that games have been used as a physical and mental rehabilitation tool with great success amongst all the negative news surrounding them.

Video gaming has opened some incredible doors to us all, and although there can be some negative effects (as with everything) they?re one of the greatest stepping stones of our time.
